Early ketogenic diet helps in some epilepsy types

NEW YORK (Reuters Health) - Patients with refractory epilepsy and certain genetic etiologies are "surprisingly responsive" to a ketogenic diet (KD), Australian researchers report.

The study also found excellent responses in patients with structural etiologies due to malformations or acquired pathology.

Most studies looking at KD in epilepsy have evaluated response based on seizure type, Dr. Mark Mackay of Royal Children's Hospital in Parkville, Victoria and his colleagues said in a February 6 "Brief Communication" in Epilepsia.

To better understand whether the KD should be initiated earlier in patients with certain syndromes or etiology, rather than waiting until other treatment options have been exhausted, Dr. Mackay and his team prospectively enrolled 64 children and four adults and started them on the diet between 2002 and 2009. All had refractory seizures, based on failure of at least two anticonvulsants

Seven children were excluded from the analysis; in five cases, this was because their families failed to comply with the dietary protocol.

In the remaining 61 patients, the median age at seizure onset was three months, and the median age at KD initiation was 4.8 years.

Fifty-three patients had more than one seizure type. In the 42 patients for whom etiology was determined, 18 had genetic causes, 11 had developmental malformations, and 13 had acquired abnormalities.

The KD was initiated in all patients after hospital admission, with dietary ratios ranging from 2:1 to 4:1 fat to carbohydrate and protein.

At three months, 29 patients (47.5%) had responded, meaning their seizure frequency was reduced by more than 50%. One child with focal epilepsy of unknown etiology and another with refractory childhood absence epilepsy became seizure free.

Syndromes that responded to the KD included migrating partial epilepsy of infancy (two of two patients); childhood absence epilepsy (two of two patients); focal epilepsy (two of two patients), epilepsy with myoclonic-atonic seizures (two of three patients) and Dravet syndrome (two of four patients).

Etiologies with "surprisingly good responses," according to the authors, included lissencephaly (three of four patients) and hypoxic ischemic encephalopathy (four of eight patients).

At present, the researchers say, the KD is known to be effective in myoclonic epilepsies. "Small numbers of patients with specific etiologies prevented us from drawing firm conclusions about the suitability of the diet for individuals with a specific electroclinical syndrome," they add. "However, our data suggest that patients with hypoxic-ischemic encephalopathy and lissencephaly should be considered earlier for this nondrug therapy."

They conclude. "Larger studies of specific syndromes and etiologies will provide more definitive insights in to the suitability of the KD."

The study was supported by a grant from Pfizer Pharmaceuticals.
